Documents needed to show your player either lives within the CHLL boundaries OR attends a school within CHLL's boundaries.

The "Proof of Residency" documents can be tricky.  Little League requires a total of THREE (3) different documents to prove you live within CHLL's boundaries.  One document from each of the 3 "Groups" listed on the form ( the same document cannot be used in more than one Group).  All 3 documents must show an in force date between February 1st of the previous year and January 31st of the current year.

The "School Enrollment" form is the easiest way to prove your child goes to school within CHLL's boundaries!  The School Enrollment form is to be submitted to your child's school, and can only be signed/completed by the school Principal or selected school Official.
